Consent to the processing of personal data

I hereby give my consent to the processing of my personal data in the form of: first name and surname; street; house number; apartment number; postal code; city; country; contact phone number; e-mail address; company name; tax identification number (NIP), for the purpose of providing services to me by the Controller, Keten Group, with its registered office at ul. Puszkarzy 7, 04-436 Warsaw, NIP: 9522254388, REGON: 528242970.

I hereby declare that, pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), I have been informed that:

My personal data will be processed solely for the purposes indicated above and will be disclosed to third parties, with the recipients of the data being entities providing services to the data controller – on the basis of data processing agreements
My personal data are processed for the purpose of performing the contract concluded between me and the controller, as well as on the basis of my consent — for direct marketing purposes.
My personal data will be stored for the duration of the agreement concluded between me and the data controller.
I have the right to request from the controller access to my personal data, as well as its rectification, erasure, or restriction of processing. I also have the right to object to the processing and the right to data portability.
I have the right to withdraw my consent to the processing of my personal data without affecting the lawfulness of processing carried out on the basis of my consent before its withdrawal.
I have the right to lodge a complaint with the supervisory authority.
Providing personal data is voluntary, and failure to provide it will result in the inability to perform the contract concluded between me and the data controller.
The administrator collects data in an automated manner but does not apply profiling to me.
I declare that I am of legal age and have full legal capacity. I also declare that I have read the full content of the above consent and fully understand it.
